2011-2012 Senior Portfolio Requirements
ACT/SAT & Financial Aide Documentation
For this section, you must insert a copy of your ACT or SAT score report and evidence that you have sent in a FAFSA form. Please attach a reflection form where you comment on your feelings about your score and how that score will impact your future.

If you are not planning on attending college, and you have not taken the ACT or SAT, please insert a copy of an armed services commitment letter or a letter from an employer who has hired you to work after graduation. If you are applying to a technical program or a trade apprenticeship, please include a copy of your application. Please attach a reflection form to these items.

Transcript Evaluation



For this section, please insert an actual copy of your current transcript. Attach a reflection form where you comment on your feelings about the grades you have earned and how these grades will impact your future. Refer to the jury questions for assistance.

Letters of Acceptance/Job or Military Commitment
For this section, please insert any college acceptance letters you have received. If you are not planning on attending college, please insert either a letter of commitment from an employer or the military. If you do not have any of the above, explain why in a one-page essay.

Best Work



For this section, please insert six pieces of your “best work.” You should have one piece from each core area (English, math, science, social studies) and two pieces from any other areas (Spanish, art, music). Pieces must show higher order thinking skills and CANNOT include basic items such as bell ringers, exit tickets, worksheets, etc. Acceptable items include essays, projects, presentation artifacts, literacy activities, etc. You must attach a reflection form to each piece that is approved by a teacher from the subject area in which the piece was completed.

Career Evidence



For this section, please insert four pieces of evidence from career experiences in which you have participated. These experiences could include guest speakers, job shadowing, internships, work experience, field trips, etc. A specific format to follow while documenting your experiences will be given to you by your career teacher. You must attach one reflection form to each of these pieces. The reflective writing for these experiences should be especially strong since they are in your chosen small school content area.
